Temporary Clinical Case Manager - SFDPH

Job Overview

The Temp TAP Clinical Case Manager is temporary, not to exceed 8 months. As part of an interdisciplinary team, the TAP Clinical Case Manager, licensed, works with staff to extend effective harm reduction-oriented services to clients of the program. The clinical case manager provides collaborative, client-centered clinical services throughout the various stages of engagement which includes assessment to determine appropriate level of care; crisis intervention; treatment planning; client education; supportive individual and group therapy; maintaining accountability and consistency; demonstrating excellent follow through with established plans; developing and maintaining effective working relationships with a variety of community service and treatment agencies; and assisting with arranging translation, transportation and other practical support services. Other comparable duties may be assigned as well.

The CBHS, Treatment Access Program is a collaborative effort between healthRIGHT360 and the San Francisco Department of Public Health, San Francisco Health Network, Behavioral Health Services (BHS) to divert eligible individuals into appropriate behavioral health treatment services, with an emphasis on clients with substance use disorders. TAP is the centralized access point for substance use services for the City and County of San Francisco. This position requires an ability to maintain strict confidentiality and handle information with tact and diplomacy.

 

Key Responsibilities

  • Initial screening of clients for triaging into appropriate level of care.
  • Provide direct services to adult clients, including intake, behavioral health assessment, treatment planning, individual and group therapy, case management, liaison with community agencies and information and referral
  • Administering clinically driven assessments, including, but not limited to, the Addiction Severity Index (ASI), the Clinical Institute withdrawal Assessment of Alcohol Scale, Revised (CIWA-Ar), the Clinical Opiate Withdrawal Scale (COWS).
  • Perform Officer of the Day and Detox Authorizer duties at regular intervals
  • Provide clinical leadership
  • Managing psychiatric and substance use crises
  • Case conferencing new cases and ongoing engaged clients
  • Engaging in field assignments on a regular pre-determined and ad hoc basis
  • Completion of clinical documentation in a timely manner, engaging outside providers & providing ongoing, short-term case management
  • Provide education to clients, their families and other support regarding mental health and substance use disorders
  • Maintaining Protected Health Information in accordance with regulations and policies
  • Other duties as assigned​

Education and Knowledge, Skills and Abilities

  • Master's Degree in social work, counseling or psychology
  • Unrestricted license to practice in California (Board of Behavioral Sciences)
  • The incumbent shall work well under pressure and able to meet multiple and sometimes competing deadlines
  • The incumbent shall at all times demonstrate cooperative behavior with colleagues and supervisors
  • Knowledge of San Francisco's public health system of care
  • Cultural competence and familiarity regarding race, ethnicity, gender, sexual orientation, economic status, homelessness and chronic substance using culture
  • Demonstrated experience working with dually diagnosed clients, knowledge of evidence based practices
  • Team-oriented, critical thinking, ability to negotiate, prioritize and integrate new learning experiences
  • Familiarity with some basic anatomy & physiology concepts pertinent to the field of addiction treatment
  • Basic English proficiency, written and oral
  • Basic office skills including computer proficiency in word processing, email, and spreadsheets

Compensation: $50,000.00-$67,000.00
