Asian American Recovery Services (AARS) is looking for a committed, client-centered Care Coordinator or Care Coordinator III who is passionate about working with clients who are re-integrating into the community following incarceration. The Care Coordinator provides Cognitive Behavioral Therapy for substance use disorder and mental health treatment based on individualized assessment. Clients in this program range from 18 years old and over.

Key Responsibilities

  • Facilitates intakes, comprehensive assessments, individualized treatment planning, crisis intervention, case management and group and individual counseling/education sessions with clients
  • Conducts community outreach and educational workshops 1 Saturday, each month
  • Attends required trainings and meetings. Plans and facilitates client celebrations and special events
  • In addition to providing services at agency main site, services may be provided at the Santa Clara County Re-Entry Resource Center
  • Will adhere to documentation policies and procedures set forth by internal and external regulatory standards
  • Be responsive and pro-active in all correspondences with referring parties, judges, probation and parole officers, social workers, and the Re-entry Resource Center. via treatment status reports, phone calls, faxes, etc.
  • Will assist in providing written progress reports/narratives to Clinical Program Manager or Directors as requested

Education and Knowledge, Skills and Abilities


  • Master’s Degree in Counseling, Social Work, or related field with Supervision hours available!
  • First Aid Certified within 60 days of employment
  • CPR Certified within 60 days of employment
  • Valid California driver’s license
  • Must not be on active parole or probation
  • Knowledge of cultural stigmas of counseling and treatment services
  • Knowledge of principles and techniques of social work, case management, crisis intervention, client centered counseling, and group facilitation
  • Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data
  • Excellent organization skills and ability to multi-task and juggle multiple priorities


  • Ideal candidate will also have extensive experience with substance abuse treatment including certification/registration with a Drug and Alcohol Certification program recognized by ADP
