A Program of HealthRIGHT 360
HealthRIGHT 360 (HR360) is contracted with CalOptima Health (CalOptima), a DHCS Medi-Cal managed care plan for Orange County, to provide Enhanced Care Management services to adults transitioning from incarceration. Enhanced Care Management (ECM) is a whole-person, interdisciplinary approach to care that addresses the clinical and non-clinical needs of high need and/or high-cost Members through systematic coordination of services and comprehensive care managements that is community-based, high-touch, and person-centered.
ECM Care Managers provide assessment and screenings, linkages and navigation to primary care/BH services/social supports, appointment management/support, comprehensive transitional care, comprehensive care planning, member and family supports, and benefit enrollment services to ECM clients. ECM Care Managers may also accompany clients to appointments, as needed.

Monday – Friday, 8am-4:30pm
Eligibility for services
HR360’s target population for ECM services are adult CalOptima Medi-Cal members in Orange County who are transitioning from incarceration. Other ECM eligible populations include adults who: are experiencing homelessness, are at risk for avoidable hospital or emergency department utilization (formerly known as “high utilizers”), and/or with SMI and/or SUD needs.
